FEE STRUCTURE

Annual Renewal Fee for chiropractors practicing in the State of WV is $200.

Annual Renewal Fee for chiropractors practicing out of state or retired is $100.

If the license renewal requirements are postmarked after July 1, there is a $100 late fee.

If the license renewal requirements are postmarked after July 31, there is an additional $100 reinstatement fee.

Application for filing Articles of Incorporation is $100 per request.

The initial filing fee for a Professional Limited Liability Company is $100 AND an annual renewal fee of $100.

The exam fee is $150 per applicant.

Re-exam fee is $50 per applicant.

License verification fee is $5.00 per chiropractor.

Fee for photocopies is $.75 per copy.

The initial licensing fee is $25 per licensee.

Requests for approval of continuing education courses is $35 per course.

Application for Preceptorship is $50 per student.

Address Listing fee is $250 per list.
